Letter to Parents

This is a reminder that all students entering 7-12th grades are required by law to have a Tdap vaccination, otherwise known as Pertussis or Whooping Cough, on file with our office prior to starting the first day of school.  The vaccine is available at the Humboldt County Department of Health, Open Door Community Health and Eureka Pediatrics.  Please be sure our office has a record of this booster shot to avoid your student being excluded from school.

Also, if your student plans on playing a sport this year, you will need to complete a Sports Physical Packet and have it on file in our office prior to tryout dates.  We have volleyball and soccer tryouts the first week of school.  The packets are available in the office.  The actual physical must be completed on our insurance form, except those done at Eureka Pediatrics.

Finally, watch your mail for a New Student Packet.  Every student, new or returning, will need to completely fill out the information and bring this packet to your designated day.  6th and 7th graders will come on Monday, August 19th between noon and 4p to turn in their packet and pick up their schedule and books.  8th graders will come Tuesday, August 20th between noon and 4p.
